Self-pay rates:
Initial psychiatric evaluation: $150 · 60 minutes
Medication-management follow-up: $75 · 30 minutes

After booking through Headway
Sign in to Headway and complete any open tasks, including insurance and payment information, required forms, and identity verification if requested. Follow the deadlines shown in your account and Headway’s emails. Incomplete identity verification can cause an appointment you booked to be canceled. For account or verification problems, contact Headway support through your account. For scheduling questions, call 602-663-5580. Please do not email insurance cards, payment details, identification documents, or medical records.
Your Right to a Good Faith Estimate
If you do not have health insurance or are not using insurance for your care, you have the right to receive a written Good Faith Estimate of expected charges when you request one or schedule care.
If you schedule at least 3 business days ahead, we must provide the estimate within 1 business day after scheduling. If you schedule at least 10 business days ahead, we must provide it within 3 business days after scheduling. If you request an estimate before scheduling, we must provide it within 3 business days of your request.
If a bill from a provider or facility is at least $400 more than that provider's or facility's estimate, you may be eligible to dispute it. Keep a copy of your estimate.
